Insurance Billing for Auto Injury

Your PIP (personal injury policies) will cover treatment(s) for massage during your recovery process. Treatments begin as soon as 48 hours after your auto accident. Be sure to see your physician and bring your prescription or any medical notes with you for massage. It's not necessary to have a prescription in order to receive treatment, but it certainly does not hurt your case.

Bring your claim number, auto adjuster contact information including name, address, fax/phone and date of injury. Also if you have retained an attorney, their company name, address, phone/fax and name of legal assistant.

Please complete a health history and billing information form located under the "Client Forms" tab and bring with you to your first session or you may fax forms to 801.337.3967. For your convenience, these forms are also available in office for you to fill out prior to your appointment. Please arrive 10 to 15 minutes early to complete paperwork.
